Improving Health, Reducing Cost
MedZed partners with health plans to address the complex health-related social needs of high-risk, hard-to-reach members who have not responded to traditional outreach efforts.
We achieve this by building trusting relationships with members and enabling them to engage in their own care and improve their health and well-being. We are passionate about connecting deeply with our members to understand them, their challenges and the drivers of their health, and what will work for them. In doing so, we support health plans in providing care where it's needed most and reducing inappropriate utilization.
We collaborate with plans to identify a target population and determine key goals for Medzed’s extra layer of support. We find and enroll members, pinpoint their needs, and develop a comprehensive program of interventions.

CHNs work closely with the members, serving as the primary point of contact and coordinating all aspects of the customized care plan. They connect members with clinicians, help them access social and community services, and provide individual and family support and education. Additional specialized services and staff are also available to assist with key goals and priorities, e.g. nurses for specific clinical areas/quality metrics and housing navigators for housing resources and supports
CompleteConnect meets Medicaid Health Homes/Enhanced Care Management program requirements.
